Difficulties with emotion regulation (DERS) are common in individuals with post-traumatic stress disorder (PTSD). Often these difficulties manifest in dangerous behaviors including hazardous substance use, self-injury, aggression, and suicidality. Some clinicians are thus hesitant to initiate a trauma-focused treatment like Cognitive Processing Therapy (CPT) with clients evidencing significant emotion dysregulation, believing that trauma therapy may increase symptoms or even cause harm. Evidence suggests, however, that CPT is generally safe and effective in reducing both PTSD symptoms and difficulties in emotion regulation, even among those who exhibit dangerous emotion-driven behaviors. This chapter describes integrating elements of Crisis Response Planning (CRP) into CPT to best manage dangerous emotion-driven behaviors during treatment. Recommendations are illustrated in a case example of a combat-veteran with gun access who is experiencing thoughts of firearm suicide.
