Revascularization Strategies in HBR Patients: The Selection of Coronary Stent

In the setting of high-bleeding-risk (HBR) patients undergoing percutaneous coronary intervention (PCI), the selection of coronary stents plays a pivotal role, particularly in the choice of the adequate antithrombotic regimen. Throughout the evolution from bare-metal stents (BMS) to contemporary drug-eluting stents (DES), advances in polymer technology and stent design have been key elements in allowing the reduction of the undesirable consequences of prolonged dual antiplatelet therapy (DAPT) while maintaining efficacy in preventing the risk of stent thrombosis. A growing body of evidence from randomized clinical trials, regulatory studies, and large prospective registries has contributed to providing information regarding the safety and efficacy of various stent types in HBR patients, including durable polymer, polymer-free, biodegradable polymer, and ultrathin strut platforms. Here, we aim to provide a comprehensive review of the available data to guide clinicians in navigating the landscape of coronary stent selection for HBR patients, with a focus on practical and clinical implications aligned with bleeding avoidance strategies for the reduction of DAPT advocated by international guidelines.
