This chapter explores the impact of social determinants of health (SDoH) and social factors, such as social contact and loneliness, on health outcomes and wellbeing. SDoH, including education, control, housing, and poverty, influences about 80% of health outcomes, surpassing the contribution of healthcare services. A new individual-level SDoH measure is presented, focusing on self-reported social conditions to complement national indices like the Health Index for England. Loneliness and social contact are examined as key factors affecting mortality, wellbeing, and patient outcomes. Positive social contact fosters companionship, emotional connections, and practical support, while loneliness contributes to adverse health outcomes. The chapter introduces concise measures of how people live, developed through literature synthesis and user consultations. Case studies demonstrate the effectiveness of social prescribing in improving social contact, health confidence, and wellbeing, highlighting the need for tailored interventions to address social inequalities and promote better health.
