Vestibular migraine is a common and potentially debilitating cause of dizziness and vertigo. It can present in someone with a history of migraine headaches, or with migrainous symptoms, such as photophobia and phonophobia. Formal diagnostic criteria are available from the Barany Society. Motion sickness, motion sensitivity, and sensitivity to busy visual scenes (supermarket aisles, repeating patterns, intersections) are common. In addition, those with VM frequently experience aural pressure and tinnitus. History, exam, and audiometry are helpful to exclude other causes of vertigo, such as Meniere’s disease and vestibular neuritis. Low-level evidence is available to guide treatment decision-making, and most treatments are based on established paradigms for migraine headache.
