This chapter maps the evolving needs of older adults spanning chronic disease management, functional support, mental health, and social connectedness.  The chapter identifies how current service models fail to meet these requirements.  We highlight key global challenges, including multi-morbidity, caregiver burden, workforce shortages, and inequitable access to care.  We identify research and innovation priorities that place the older person at the centre of identifying and helping develop policy and practice solutions such as networks of collaboration and interoperable flow of data among team members.
