Chronic abdominal wall pain presents a significant challenge in both diagnosis and management, necessitating a comprehensive understanding of the chronic disease processes involved. This chapter aims to provide an in-depth overview of chronic abdominal pain, with a focus on post-herniorrhaphy neuralgia (PHN), anterior cutaneous nerve entrapment syndrome (ACNES), xiphodynia, rectus sheath hematoma (RSH), slipping rib syndrome (SRS), abdominal wall endometriosis, and oblique strain. Chronic abdominal pain encompasses a wide array of symptoms that vary depending on the underlying disease process. From localized pain experienced in rectus sheath hematoma to the persistent, chronic abdominal pain observed in ACNES, understanding the distinct symptomatology associated with each condition is crucial for accurate diagnosis. Physical examination findings and diagnostic considerations specific to each disease process are explored while outlining recommended diagnostic procedures that have demonstrated utility in identifying these disease processes. Finally, we briefly review current management strategies, both pharmacological and non-pharmacological, that have shown efficacy in alleviating symptoms and improving the quality of life for affected individuals.
