Collagenase Injections: Evidence-Based Outcomes

In this chapter, the evidence is discussed regarding outcomes of Dupuytren’s contractures treated with collagenase Clostridium histolyticum (CCH). The efficacy and safety of CCH has been confirmed with multiple studies, several of which are discussed. The reported rate of severe adverse events is low, although the minor adverse events are common. CCH has been shown to be safe for the treatment of multiple affected digits concurrently, as well as for treatment of recurrent contractures. CCH has shown successful results, with varying efficacy reported depending on length of follow-up, patient factors, and the joint treated. CCH is more successful for treatment of MCP joints than PIP joints. Compared to alternate treatment modalities, CCH has similar efficacy to surgical fasciectomy, albeit with a higher recurrence rate. Published evidence argues that CCH has roughly equivalent efficacy to percutaneous needle fasciotomy. CCH is a useful treatment option for hand surgeons in the treatment of Dupuytren’s contractures, but it is important for hand surgeons to understand the risks and limitations of its use.
