The Normal Anatomy of the Aortic Valve Apparatus

The aortic root is a complex anatomical structure connecting the left ventricular outflow tract to the ascending aorta, comprising the sinuses of Valsalva, interleaflet triangles, and semilunar cusps. Its anatomy dynamically changes during the cardiac cycle. The root features three rings: the basal ring, representing the outlet from the ventricle; the anatomic ring, shaped by the cusp attachment; and the commissural ring at the sino-tubular junction. Interleaflet triangles, located between cusp attachments, interact with adjacent cardiac and pericardial structures and serve as landmarks for key anatomical features such as the His bundle. The semilunar cusps consist of three layers—ventricularis, fibrosa, and spongiosa—ensuring flexibility and proper closure during diastole. Coronary arteries arise from the sinuses of Valsalva, typically at the sino-tubular junction. Bicuspid aortic valve, the most common congenital cardiovascular anomaly, occurs in 1–2% of the population and often involves fusion of the left and right cusps. Complications of bicuspid aortic valve include stenosis, regurgitation, endocarditis, and aortic aneurysm or dissection, underscoring the clinical importance of understanding aortic root anatomy.
