The Docking Zone in Stent Grafts Deployed Transrenally: Expression of Fibrinolytic Activators and Inhibitors in the Internal Capsule Lying over the Aorta

The Latecba stent graft was modular in design and constructed with a series of Palmaz-type stents, supporting a single jersey weft-knitted fabric to maximise the potential for tissue incorporation of the endograft. This design minimises ongoing aortic neck dilatation and prevents migration. Twenty-nine devices were successfully deployed transrenally in dogs to recanalise an infrarenal prosthetic aneurysm. All animals survived and all devices were patent at explantation. The landing proximal zones featured a healthy tissue capsule with evidence of an adequate deployment vis-à-vis the renal artery ostium and biointegration. The flow surface of the stent graft had endothelial tissue formed and displayed the expression of the von Willebrand factor (vWf). However, the proximal bare stents established small amounts of localised thrombi. The middle section of the devices demonstrated evidence of scattered thrombi. Both the bare stents and the graft fabric within the aneurysm sac did not show any biointegration. The fabric conduits were slightly dilated, questioning the possibility of future Type III endoleak. The distal landing zone showed a variable morphology depending on the degree of over dilatation in the deployed stent. The concept of a single jersey weft-knitted fabric proved to be inadequate to guarantee the biofunctionality and the durability.
