The most common and significant painful disorder in this group is classical trigeminal neuralgia (TN), typically caused by neurovascular compression (NVC). Key information that the referring headache specialist needs from magnetic resonance imaging (MRI) includes the following: (1) detection of NVC, identification of the offending vessel (artery or vein), and precise localization and grading of NVC; (2) identification of other potential causes of TN, such as tumors or demyelinating lesions; and (3) identification of patients suitable for microvascular decompression (MVD). Emerging MRI techniques and research fields are showing promise in elucidating pathological changes within the trigeminal nerve (n.V) itself and in understanding how the brains of TN patients adapt to chronic pain. This chapter also explores other painful cranial nerve lesions, which, although less common, pose considerable radiological challenges.
