Eye-related complaints are a common presentation to the emergency department and primary care setting. However, examination of the eye can be challenging for the clinician given the limitations of direct ophthalmoscopy. Point-of-care ultrasound (POCUS) has emerged as a useful tool for ocular assessment, especially of the posterior chamber. The eye is a superficial structure that is well-suited for sonographic imaging. Ultrasound is a safe, noninvasive way to assess for a broad variety of ophthalmologic pathology, including retinal detachment, posterior vitreous detachment, vitreous hemorrhage, lens dislocation, ocular foreign body, and ocular trauma. It is also a sensitive technique for identifying elevated intracranial pressure by measurement of optic nerve sheath diameter and assessing for optic disc elevation. For advanced users, it may be a modality to diagnose retrobulbar hematoma, ocular infection, ocular mass, and central retinal artery occlusion. In a busy emergency department or primary care setting, ocular POCUS can be useful for establishing an accurate diagnosis and triaging for appropriate ophthalmology referral. In a clinic setting or resource-limited environment, POCUS may be the only imaging modality available.
