Age-related descent of the brow occurs as a result of facial soft-tissue and bony volume loss, decreased elasticity of the overlying soft-tissues, and gravitational consequences. Many surgical and non-surgical techniques exist to address the brow position. The position of the brow can be addressed through a standard upper blepharoplasty incision via internal browpexy. Transblepharoplasty browpexy offers the advantage of improving brow position by elevating and stabilizing the brow without using separate incisions when concomitant upper eyelid blepharoplasty is performed. A comprehensive understanding of the anatomy of the periorbital region is key to achieving excellent results, as is identifying the patient’s aesthetic goals. Complications, though rare, can be vision threatening if not appropriately addressed. A case presentation and a detailed discussion of the surgical technique are demonstrated.
