Dermal filler-induced blindness or vascular obstruction is a rare occurrence but is a catastrophic complication both for the patient and the physician. With the rise in popularity of volumizing treatments for facial rejuvenation, these issues are being encountered more frequently. There are treatment protocols available to deal with this but with limited potential to reverse the visual loss once it has taken place. For hyaluronic acid-based fillers, hyaluronidase remains the cornerstone of treatment but that has limited effect on poly-L lactic acid (PLLA) and calcium hydroxyapatite (CaHA) fillers. Injectors should be able to recognize impending complications and deal with them expeditiously if vision loss is encountered.
