Acute dyspnea, also known as shortness of breath, is a subjective feeling of lack of air, representing a diagnostic challenge even for the experienced physician. The differential diagnosis is complex, including a wide spectrum of diseases categorized into obstructive, parenchymal, cardiac, neuromuscular, psychogenic, systemic illness, or a combination of these. This chapter is focused on the main non-traumatic thoracic diseases causing acute dyspnea, on the relative imaging features, and on the difference imaging modalities, employed in emergency settings.
