This chapter discusses the most serious component of drug use, the adverse drug reaction (ADR). Adverse drug reactions are high in the older population. Whereas there are many reasons for this, the focus is on polypharmacy prevalent in older adults with multiple morbidities. The resulting adverse drug–drug interactions are discussed at the pharmacokinetic and pharmacodynamic level. Adverse drug reactions can be avoided. Successful strategies that provide guidance to achieve this include the Beers Criteria® Guide to Potential Inappropriate Medications, STOPP/START guide, and deprescribing strategies. These are discussed in detail. Guidance is given that summarizes key points to enhance better health outcomes.
