This chapter is the author’s personal reflection upon 30 years of international cleft care volunteerism focused on assisting local healthcare providers and institutions in developing self-sustaining cleft care centers and assisting their maturation. The author discusses whether palatoplasty and VPD management differ between healthcare resource-rich and resource-limited environments with respect to timing of identification of the defect, age at management, resources for management, complication management, longitudinal care, education for the affected individual/family, outcome measures, whether to operate at all, indications for surgery, choice of procedures, rationing of services, compensation for cleft care deficiencies, and whether there are similarities in cleft care challenges between resource-rich and resource-limited healthcare environments.
