Clostridioides difficile is an important cause of both hospital- and community-acquired infectious colitis. Infection severity ranges from mild diarrhea to fulminant hemorrhagic colitis resulting in shock and death. Older adults are at highest risk for C. difficile infection but C. difficile also affects pediatric patients. Recent antibiotic use, immunocompromise and chronic conditions, like inflammatory bowel disease are known to increase a person’s risk of infection. Because asymptomatic colonization is common, a multi-step testing algorithm is required to distinguish between colonization and pathogenic infection. Fidaxomicin is the treatment of choice for adults with C. difficile infection while oral vancomycin is the preferred treatment in children. For fulminant infection, oral vancomycin is recommended regardless of age.
