Total hip arthroplasty (THA) has been called the operation of the century. Patient satisfaction is reported to be up to 96%. A reported 50,220 revision THA (rTHA) surgeries were performed in 2014. From 2014 to 2030, the incidence of revision THA is projected to increase by between 43% and 70%. As the number of revision THAs continues to rise, it is important to be able to evaluate and identify potential causes of failure to ensure adequate treatment. This chapter will include pertinent history taking, physical examination, and laboratory assessment of the painful or failing THA. While certain etiologies of failure are straightforward in presentation, a methodical and complete approach to evaluation can allow the treating physician to be both secure in the diagnosis and confident in the formulation of a revision THA, if and when it is required.
