In addressing an acetabular defect, it is critical to have an intimate understanding of the relevant anatomy and etiology of failure. Preoperative planning is key, and utilizing a scheme, such as the AAOS or Paprosky classification, is helpful in describing acetabular deficiency and planning for revision surgery. These problems are daunting and pose a significant burden on the patient, surgeon, and healthcare system. However, nearly all cases can be effectively addressed when paired with a correctly indicated surgical technique. In this chapter, the popular evidence-based reconstructive options will be presented, including hemispherical cups, reconstruction rings, structural allograft, modular porous metal augments, cages, and custom triflange acetabular components.
