Large acetabular defects and pelvic discontinuity are dreaded complications from osteolysis, corrosion, implant loosening, or fracture around a hip replacement acetabular component. There are numerous methods to address these issues, often dictated by the nature of bone loss and defects present. The presence of large superior medial defects, especially if acetabular component protrusion or discontinuity exists, makes salvage difficult. Numerous reconstruction options exist for these scenarios, including jumbo hemispherical acetabular cups, custom triflange components, and cup-cage implants. This chapter discusses the cup-cage procedure in detail, including technical pearls and relevant outcomes for this mode of acetabular reconstruction.
