Introduction

In 2022 I published a Primer focused on the clinician educator, those academic faculty in medicine and health sciences that assume the major responsibility for teaching, patient care, educational scholarship, advocacy and mentoring in our academic health centers. That publication triggered an idea for this book; i.e., reflecting on mistakes clinician educators make in patient care and with their educational responsibilities. Historically, there have been just a few publications that have addressed mistakes specifically made by physicians in patient care, the most important of which was the 1999 Institute of Medicine’s treatise To Err is Human: Building a Safer Health System, which recognized how errors impact the safety of patients.
