This is a case with metastatic melanoma who developed insulin dependent T1DM with DKA upon treatment of epacadostat and durvalumab in the context of pre-existing T2DM. The patient concluded cancer therapy with continued insulin requirement to maintain a stable hemoglobin A1c around 8%. He has since remained without recurrence of cutaneous disease until the last follow up for 7 years after pancreatic endocrine toxicity event.
