This is a case with bilateral breast cancer who underwent treatment with pembrolizumab and subsequently developed LVV. She underwent several lines of treatments including tocilizumab and 2 courses of cyclophosphamide. In addition, she required continuous steroids and was unable to wean below 10 mg of prednisone daily. Despite this, she had ongoing aortitis and enlarging proximal aortic aneurysm that ultimately underwent surgical repair. Histologic examination of aortic tissue demonstrating marked granulomatous inflammation.
