This is a case with Merkel cell carcinoma, received 6 doses of avelumab with subsequent severe necrotizing pancreatitis. The disease course after steroid treatment was complicated with pancreatic abscess/fistula and sepsis requiring prolonged antibiotics, frequent drain placement and long term TPN. Patient ultimately recovered from the event after 1 year. Over the follow up of 2 years, patient developed asymptomatic pancreatic atrophy, with cancer in remission despite ICI termination.
