We presented a case with metastatic lung adenocarcinoma who developed ICI-induced erosive oral lichen planus with a component of pemphigus vulgaris. Biopsy results demonstrated a lichenoid dermatitis with abundant colloid bodies along a subepidermal split, while direct immunofluorescence staining revealed immunoglobulin deposition between the epidermal keratinocytes. The patient responded well to corticosteroid treatment initially, but ultimately required methotrexate and mycophenolate mofetil for long-term clearance.
