Rectal Ulcer and Perianal Fistula in a Cancer Patient on Immune Checkpoint Inhibitor Therapy

This is a case with metastatic renal cell carcinoma on nivolumab/ipilimumab and cabozantinib who presented with progressively worsening diarrhea and was found to have duodenitis, rectal ulcer, and perianal fistula/fluid collection. He was treated with antibiotics, budesonide, and 3 doses of vedolizumab with significant symptom response and resolution of fistula on follow up imaging 3 weeks later. He was successfully resumed on cabozantinib and remained on nivolumab during the entire GI event.
