This is a patient with stage IVB esophageal squamous cell carcinoma who received eight cycles of pembrolizumab and subsequently developed pouchitis in the J-pouch from IPAA surgery. His prolonged disease course was complicated by diarrhea and rectal bleeding requiring hospitalizations and treatment with both steroids and vedolizumab. He achieved sustained clinical and endoscopic remission after treatment, and his cancer has remained stable off pembrolizumab and any other systemic chemotherapy for 2.5 years of follow-up surveillance.
