This is a case report of a woman with metastatic triple negative breast cancer who had received two cycles of chemotherapy and pembrolizumab who subsequently developed fever with CT findings of central midgut mesenteric soft tissue stranding concerning for SM. This patient was treated with a short course of corticosteroids and had resolution of her symptoms and no further recurrence of SM. The patient was able to resume her previous cancer therapy including immune checkpoint inhibitor therapy.
