We present a case with metastatic urothelial carcinoma on ICI treatment, then developed multi-organ toxicities with complicated disease course of CMV pneumonitis and colitis, and bowel perforation post immunosuppressive therapy. The patient eventually passed away from multi-organ failure despite CMV treatment, and prompt surgery for perforation.
