We described a case with recurrent squamous cell carcinoma of the head and neck who developed significant persistent unexplained symptomatic episodic hypoglycemia on pembrolizumab monotherapy. A comprehensive workup including endocrinological and autoimmune panel as well as imaging to look for insulinoma were all negative. Withholding pembrolizumab and prednisone treatment achieved normalization of blood glucose and resolution of symptoms.
