Approaches to Maxillary Sinus

Many different approaches can be taken for lesions of the maxillary sinus. The approach is tailored to the specific characteristics of the tumour and the outcome required. Broadly, they are divided into endoscopic approaches, open approaches and maxillectomy. In the past three decades, endoscopic sinus surgery has replaced open sinus surgery as the standard of treatment. Many different endoscopic techniques exist, such as middle meatal antrostomy, maxillary sinusotomy, large antrostomy, total endoscopic medial maxillectomy, modified endoscopic medial maxillectomy, trans-septal approach and modified Denker approach. The open approaches include the Caldwell-Luc approach, lateral rhinotomy incision, bone removal (medial maxillectomy), extended lateral rhinotomy approaches, midfacial degloving approach, Weber–Fergusson approach and maxillectomy.
