Faecal Volatile organic compound (VOC) analysis holds promise for screening and surveillance of advanced colonic neoplasia, with reported sensitivity and specificity both up to 100% for colorectal cancer (CRC) and 97% and 94% for advanced adenoma, respectively. Implementation of faecal VOC profiles in screening programmes may lead to earlier detection and treatment of relevant lesions, possibly reducing morbidity and mortality rates, endoscopy overuse, patient burden and health care costs. In this chapter, an overview of the literature on this topic is given.
