Cancer field surgery for the locoregional tumor control of carcinomas of the uterine cervix and vagina is described step-by-step in an optimized sequence. The procedures, total and extended mesometrial resection (TMMR and EMMR) and immunologic defense line-directed lymph node dissection (iLND), are performed through open abdominal or abdominoperineal access. The procedural details are supplemented with notes on the preoperative management and presentation of optimal surgical instruments. Modifications of the standard techniques for certain pre- and intraoperative situations are specified. Recommendations for postoperative management, both in the uneventful course and in case of complications, are given.
