Anatomical reconstruction with local skin flaps is considered an essential element in the treatment of vulvar carcinoma following cancer field surgery. The goal of anatomical reconstruction is the restoration of the main form elements of the vulva and its function, including sensation. Extended surgical treatment for regional tumor control also demands protective soft-tissue covering. A variety of random and axial pattern skin flaps as well as musculocutaneous flaps developed by plastic surgeons can be successfully applied in these instances. The procedural details of a set of four flaps modified and specified by the author are described: perianal Limberg biflap, paralabial island flap, paralabial peninsular flap, and gluteopudendal thigh flap. Infectious wound-healing complications due to the naturally septic perineal field and management options are discussed.
