Cancer field surgery for the locoregional control of vulvar carcinoma, vulvar field resection (VFR) with immunologic defense line-directed lymph node dissection (iLND), is illustrated step-by-step. The local procedures of various extensions include total, partial (anterior, posterior, lateral), focal, and extended versions. Cancer field surgery to remove the sinus vagina is considered as well. iLND encompasses the removal of sentinel, first-, second-, and third-line inguinal, respectively lacunar, lymph node regions. The presentation of the techniques is supplemented with a compilation of the optimal surgical instruments and recommendations for pre- and postoperative management.
