Electronic health record (EHR) designs can lead to unintended practical consequences that are chiefly conspicuous to end users (Colicchio et al. 2019). Though some specific domains such as usability (Melnick et al. 2020) and the EHR’s effect on workload (Overhage and McCallie 2020) have recently received increased attention, our understanding of how to best address collateral impacts of EHR design is incomplete. With the aim of grounding conversations regarding users’ experience with the EHR within a clinical context, we present a case study demonstrating how encoded workflows such as clinical pathways can potentially have unforeseen impacts on patient care.
