Update on Glioma’s Neuroradiology

The World Health Organization (WHO) classification of brain tumors in 2021 featured significant advances in molecular, genetic, and pathogenesis knowledge, which revolutionized brain tumor classification by emphasizing molecular types and separating adult- and pediatric-type diffuse gliomas. Neuroradiology, which focuses on diagnosing central nervous system (CNS) diseases, has also played an important role in the diagnosis, treatment, and management of glioma patients by providing valuable insights. Despite magnetic resonance imaging (MRI) being the primary imaging modality for primary intra-axial brain tumors, the diverse tumor characteristics among glioma patients pose a challenge for disease outcome prediction. To address this, a promising approach is to combine molecular-level information with radiological imaging. This chapter highlights recent evidence supporting the use of radiogenomic biomarkers to understand the underlying biology of gliomas and their connection to classical imaging features. Radiogenomics can offer insights into tumor biology and the molecular pathways that drive it. Certain MRI characteristics that show strong correlations with EGFR, MGMT, and IDH1 can serve as imaging biomarkers. In conclusion, this chapter focuses on the intraoperative MRI sequences used during surgery, such as functional MRI (fMRI) and tractography.
