Total knee arthroplasty (TKA) is a procedure well tolerated by patients, in which implants work well for periods longer than 15 years. The objective of this study is to explore the latest research highlighted by the specialized literature as being novelly advisable in TKA which can increase the lifespan of medical implants beyond the current average. The methodology of this study consisted primarily in the analysis of specialized literature and the extraction of the most recent papers related to novel advances in total knee arthroplasty surgery. It was explored the multimodal perioperative care protocols, minimally invasive interventions, implant performance, robotic assisted interventions, knee arthroplasty revisions, patient-specific cutting guides. Results reveal that advanced perioperative protocols are specific to healthcare facilities, but they have some common characteristics that are presented. The constant evaluation of minimally invasive surgery for TKA indicates very few clinical improvements. The knee implants have not progressed in the last two decades compared to the initial prosthetic solutions that were cemented. The surgical procedures assisted by the robot increase the duration of the intervention and reduce the cases of faulty alignment. They are more expensive, and the clinically relevant benefits justify their use. There is no consistent demonstration of the effectiveness of personalized cutting guides. We conclude that the most recent and significant advances in knee arthroplasty are related to the perioperative management of patients. This is contrary to the expectations that estimated progress in the constructive solutions of implants, in the materials from which they are manufactured or in the implantation technologies, especially those assisted by robots.
