The dynamic anterior stabilization (DAS) of the shoulder is an arthroscopic soft tissue technique that utilizes the transfer of the long head of the biceps (LHB) for the treatment of anterior glenohumeral instability in patients with minimal to subcritical anterior glenoid bone loss, with or without bipolar bony lesions, with or without anterior capsulolabral insufficiency, with or without concomitant SLAP tears. Depending on the type and severity of the concomitant lesions, the DAS can be utilized to augment the Bankart repair, to provide an anterior labroplasty effect in the absence of sufficient anterior labral tissue, to tenodese the LHB in a patient with a concomitant SLAP lesion and to augment the Hill-Sachs remplissage in the presence of a bipolar bony lesion. In the current chapter, the authors discuss the rationale, the tips and tricks and pearls and pitfalls of the surgical technique, the results, and the current indications of the DAS of the shoulder.
