Hypertension (HT) is common in most patients after kidney transplantation (KT) and might be difficult to diagnose and control. Post-kidney transplant HT might be triggered by pre-KT factors associated with the KT procedure per se and by factors occurring after KT and is associated with dysfunction and decreased survival of the graft, loss of kidney function, cardiovascular outcomes, and increased mortality. The pathogenesis of HT following KT is multifactorial and involves risk factors present before KT, risk factors associated with the KT procedure per se, and parameters present after KT and include kidney graft dysfunction, the use of immunosuppressive agents, renal artery stenosis, donor characteristics, post-transplant obesity, and sleep apnea. In this chapter, we provide an update of the clinical epidemiology, pathophysiology, risk factors, clinical consequences, and recent therapeutic advancements of post-transplantation hypertension.
