The management of bone loss in the setting of open fractures remains a challenge for orthopedic surgeons. Here, we outline an approach to patients with bone loss from open fractures. We discuss surgical techniques available for bone defects of varying sizes and provide an overview of the management of this complex orthopedic dilemma. Satisfactory outcomes can be achieved with the utilization of general orthopedic principles and a multidisciplinary approach.
