The adoption of Electronic Health Records (EHRs) is vital for digital transformation in healthcare, enhancing patient care, efficiency, data-driven decision-making, and patient engagement. Despite challenges such as cost, interoperability, data security, and user resistance, the long-term benefits make EHRs essential for modern healthcare. Türkiye faced issues with healthcare accessibility, insurance coverage, quality, efficiency, and equity before 2003. A Health Transformation Program (HTP) was launched in 2003 by the Ministry of Health (MoH) and Ministry of Labor and Social Security (MoLSS). The program aimed to improve healthcare through radical reforms, including restructuring the MoH, building health insurance capacity, reorganizing healthcare delivery, strengthening human resources, building health infrastructure, and project management. Since 2003, some improvements such as enhanced patient access, health outcomes, and satisfaction have been achieved, though regional disparities and financial sustainability issues remain. Efforts to improve efficiency and input quality in public hospitals have been noted, with a focus on financial sustainability. To standardize the Nationwide Health Information System (HIS), National Health Data Dictionary (NHDD), Health Coding Reference Server (HCRS), Minimum Health Data Sets (MHDSs), and guidelines for IT procurement and data security have been developed. For digitalization and integration, Family Physician Information System (FPIS), Sağlık-Net (Health-Net), e-Nabız (e-Pulse), Central Physician Booking System (CFBS), and other applications like teleradiology and e-prescription have been developed. For guiding and measuring adoption, the MoH used HIMSS EMRAM and O-EMRAM digital maturity models. Significant progress in EHR adoption has been achieved in most public hospitals and oral and dental health hospitals in Türkiye, with all hospitals using at least basic EHR functions since 2021. The HTP in Türkiye has notably improved healthcare quality indicators and EHR adoption, contributing to a more integrated, efficient, and patient-centered healthcare system.
