Farming holds countless dangers for its workers. The number one cause of death is traumatic in nature, specifically with the use of tractors in the United States. The amount of chemicals and machinery involved in modern-day farming, together with the remote location of farms, makes prehospital care challenging. Aside from traumatic injuries, farmworkers can be exposed to chemicals such as pesticides. About 75% of pesticides used in the United States are used in agriculture. The safety of the on-scene healthcare personnel is essential when dealing with toxic exposure. Every effort should be taken to minimize the risk of transporting a contaminated victim, thereby moving the hazardous condition to the emergency department and exposing others. The prehospital literature on insect envenomation is limited; however, management has remained relatively unchanged with priority on early recognition of anaphylaxis, supportive treatment, and early transport to an appropriate facility, regardless of the envenomation type. Although transportation to a facility that stocks and administers antivenin is of high priority, initial medical intervention should utilize the same primary survey recommended for most medical emergencies. Most toxic farm gas exposures occur in confined or open spaces. Confined spaces include grain silos, manure pits, grain bins, and controlled atmospheric storages.
