The direct impact of COVID-19 on pediatric acute care occurred not only in management of pediatric patients with COVID-19 but also included transformation of pediatric intensive care units into adult intensive care units. Pediatric healthcare professionals collaborated to formulate best treatment strategies for both COVID-19 and multisystem inflammatory syndrome in children. The indirect impacts of the pandemic negatively impacted parental presence, learner training, surgical subspecialty care, and traumatic injuries, particularly penetrating trauma and suicide attempts, which both dramatically increased. The fragility of the US healthcare system was exposed during the pandemic and has persisted.
