Intra-arterial (IA) drug delivery involves super-selective catheterization of blood vessels to deliver therapeutic agents directly across the blood–brain barrier (BBB) to target tumors. By selectively targeting the tumor’s blood supply and disrupting the BBB, high concentrations of drugs can be delivered to the tumor while minimizing systemic toxicity. This chapter reviews superselective intra-arterial cerebral infusion (SSIACI) with BBB disruption for brain tumor treatment. It further highlights areas for further investigation. These include the need to refine and standardize IA drug delivery techniques, better understand glioblastoma’s complex structure (GBM), improve drug selection and formulations, and develop methods to minimize neurological complications. Lastly, the chapter reviews ongoing clinical trials that assess IA therapy for brain tumor treatment.
