Hepatic artery chemotherapy improves survival in patients with primary and secondary organ malignancies. Many unique features of the liver enhance the therapeutic effectiveness of intra-arterial drug delivery. One reason for the success of hepatic artery chemotherapy has been the development of implantable infusion pumps that permit chemotherapy to continue in an ambulatory setting that is less disruptive to the patient’s life. Long-term drug delivery has been used for treating malignancies in other organs as well. Likely, advances in pharmacology, implantation techniques, less thrombogenic catheters, and pump technology will further improve the outcome of such treatments. This chapter introduces the reader to long-term hepatic artery chemotherapy while assessing the challenges in applying these methods for treating neurological diseases.
