Developing Arterial Drugs: Lessons from the Hepatic Artery and Other Arterial Injections

The success of intra-arterial (IA) treatment of liver malignancies reflects the advances in arterial drug delivery. Hepatic artery chemotherapy can be broadly divided into unmodified arterial infusions, conventional gel-based arterial drug delivery, particle embolization, and delivery of radiopharmaceuticals. In contrast, most applications of IA drug delivery for treating neurological diseases are based on the off-label use of intravenous drugs. Unless the drugs are explicitly developed for maximum first-pass extraction, the potential of IA drugs in treating neurological diseases will remain unrealized. This chapter seeks to learn from clinical experience with hepatic artery chemotherapy, radiocontrast agents in angiography, and complications of arterial drug injections, which could give clues for developing drugs for treating neurological diseases.
