Constrictive pericarditis (CP) is a form of diastolic heart failure with the most common etiologies being idiopathic, surgery, and radiation. It is characterized by impaired cardiac diastolic filling due to a non-compliant pericardium with hemodynamic changes due to two main physiologic effects: the dissociation of the intrathoracic and intracardiac pressures and an exaggerated interventricular dependence. Given that patients with CP frequently present with generalized right heart failure symptoms, clinical diagnosis can be challenging. Transthoracic echocardiography is an effective and noninvasive diagnostic tool to identify and differentiate CP from other disease processes such as restrictive cardiomyopathies and tricuspid valve regurgitation. Utilizing two-dimensional and Doppler echocardiography, assessment of respiration-related ventricular shift, mitral inflow velocities, hepatic vein flow reversal, and mitral annular tissue velocities can assist in the successful recognition of constrictive pericarditis.
