The Right Ventricle

Historically, the left ventricle has been the prime focus when assessing cardiac function, with the right ventricle (RV) being thought of as a passive conduit for blood to flow from the venous system to high-compliance low-resistance pulmonary circulation. The closed nature of the circulatory system requires both ventricles to work together efficiently to maintain organ perfusion with failure of one ventricle deleteriously affecting the performance of the other. The right side of the heart is very sensitive to physiological extremes and is adversely affected by acute lung injury, use of mechanical ventilation, extreme acid-base disorder, volume status, and shock states with the use of inotropic and vasoactive agents, which are conditions frequently observed in critically ill cohorts. With the increasing utilization of bedside ultrasound in the intensive care unit, critical care clinicians are increasingly recognizing the dysfunction of RV previously termed “the forgotten chamber” as an independent predictor or evident contributor to mortality in left-sided heart failure, acute myocardial infarction, pulmonary embolism, pulmonary arterial hypertension, acute respiratory distress syndrome, and patient requiring mechanical circulatory support. Appraisal of right heart parameters is important in these clinical scenarios to facilitate diagnosis, prognostication, and monitoring response to therapy. Echocardiography remains the first-line imaging investigation for right heart assessment. In this chapter, we will discuss practical aspects of right ventricular assessment using echocardiography, including qualitative, quantitative, chamber dimensions, and function during critical illness.
