The paper presents the implementation of a model of an employee evaluation system. The implementation consisted of creating a database server based on the PostgreSQL database and connecting it to the database management software. Software based on Libre Office Base (LO Base) described by Libre Giude, which is part of the Libre Office suite, allowed for the design and creation of templates for employee evaluation forms, as well as the automatic creation of forms for a larger number of employees using data from the PostgreSQL database server. The forms were created on the example of the employee evaluation system at the Czestochowa University of Technology (CUT). Open Source tools were used for the above solution. The work presents a method of connecting data from the PosgreSQL system as a source, with LO Base software, which enable comprehensive creation of forms and documents necessary for documentation required by regulations presented in Rectors’ Ordinance and Regulation of the Minister. The main motive of the research was not only to facilitate the performance of employee evaluation by research and teaching staff of the Częstochowa University of Technology but also to verify the criteria contained therein. Another reason was the lack of such software at the above-mentioned university. There is still a gap in the literature regarding the evaluation system for employees of higher education institutions in Poland that is worth filling.
